If you have supporting documentation for your complaint, such as letters or reports, we advise that you print off the complaint form which is available in two formats, a Microsoft Word document or a Adobe Acrobat Reader file and send it with the supporting documentation in one envelope to the Ombudsman for the Defence Forces.
If there is no supporting documentation you can register your complaint with the Ombudsman for the Defence Forces on-line. Complete this form and click the Send button.
Whether you choose to make a complaint on-line or by post keep a copy of the completed form for your records.
