If you have supporting documentation for your Complaint, such as letters or reports, we advise that you print off the Complaint Form which is available in two formats, a Microsoft Word document or a Adobe Acrobat Reader file and send it with the supporting documentation in one envelope to the Ombudsman for the Defence Forces.

If there is no supporting documentation you can register your complaint with the Ombudsman for the Defence Forces on-line. To do so, please complete this form and click the Send button.

Whether you choose submit a complaint on-line or by post keep a copy of the completed Form for your records.

Please bear in mind that the details of your Complaint will have to be submitted to the Defence Forces as part of the Preliminary Examination and Investigation. If you are making a complaint against a named person then that person must be given a chance to respond. By submitting this Complaint Form you are consenting to the matters you raise being made known to those involved.
